چکیده

Ischemic stroke is a syndrome, and the optimal use of an¬ tithrombotic therapies in patients at risk for ischemic stroke is guided by the specific pathogenesis (Figs 1 and 2). Patients with increased risk for ischemic stroke can be identified (Table 1). Atherosclerosis of arteries, large and small, supplying the brain is the most common cause of is¬ chemic stroke in North America and Europe. Atherosclero¬ sis of the proximal aorta is another source of atherogenic emboli recently detectable by transesophageal echocardi¬ ography. Microatheroma and other occlusive disease of the small penetrating brain arteries are the most frequent causes of small, subcortical "lacunar" infarcts. About 20% of ischemic strokes are due to cardiogenic embolism. A variety of other arterial occlusive disorders may be the primary cause or variably contribute to stroke pathogenesis. Overall, about 30% ofischemic strokes remain "cryptogenic" despite

Effects of blood pressure lowering on intracranial and extracranial bleeding in patients on antithrombotic therapy: the PROGRESS trial.

BACKGROUND AND PURPOSE Observational studies demonstrate strong associations between blood pressure and bleeding complications of antithrombotic therapy. The objective was to determine whether blood pressure lowering reduces risks of bleeding in patients on antithrombotic therapy. METHODS This is a subsidiary analysis of the Perindopril Protection Against Recurrent Stroke Study (PROGRESS) tri...

XIV. Cerebral ischemia: the role of thrombosis and of antithrombotic therapy. Study group on antithrombotic therapy.

A revival of interest in antithrombotic agents for the treatment of ischemic cerebrovascular disease has resulted in the widespread use of oral anticoagulants for the prophylaxis and therapy of the ischemic variety of stroke, and has generated enthusiasm for the use of platelet-suppressant agents such as aspirin, dipyridamole, and sulfinpyrazone. In delineating the several clinical types of foc...

Bleeding Risk Related to Upper Gastrointestinal Endoscopic Biopsy in Patients Receiving Antithrombotic Therapy: A Multicenter Prospective Observational Study

BACKGROUND Although antithrombotic agents are widely used for cardiac and cerebrovascular disease prevention, they increase the risk of gastrointestinal (GI) bleeding. OBJECTIVE To examine GI bleeding risk in association with an esophagogastroduodenoscopy (EGD) biopsy performed in patients without cessation of antithrombotic therapy. METHODS This study was prospectively conducted at 14 cent...

Clinical problems with antithrombotic therapy for endoscopic submucosal dissection for gastric neoplasms

Endoscopic submucosal dissection (ESD) is minimally invasive and thus has become a widely accepted treatment for gastric neoplasms, particularly for patients with comorbidities. Antithrombotic agents are used to prevent thrombotic events in patients with comorbidities such as cardio-cerebrovascular diseases and atrial fibrillation. With appropriate cessation, antithrombotic therapy does not inc...

Antithrombotic Therapy: Evaluation of the Safety of Performing Core Needle Biopsy of the Breast Without Suspending Medication
.

BACKGROUND Patients are increasingly presenting for outpatient breast biopsy while taking medically necessary antithrombotic therapy. Cessation of this medication prior to biopsy increases patients' risk of vascular complications.
. OBJECTIVES This article evaluates the safety of performing core needle biopsies of the breast in patients without suspending prescription antithrombotic therapy....

Carotid artery stents for blunt cerebrovascular injury: risks exceed benefits.

BACKGROUND Carotid stenting has been advocated in patients with grade III blunt carotid artery injuries (hereafter referred to as "blunt CAIs") because of the persistence of the pseudoaneurysm and concern for subsequent embolization or rupture. HYPOTHESIS Carotid stenting is safe and effective for blunt CAIs. DESIGN Analysis of a prospective database of all patients with blunt CAIs. SETTI...
